  1. ELIGIBILITY

This drawing is open to:

  • Patients aged 18 years or older
  • Parents or legal guardians of patients under 18
  • Legal caregivers of patients

NOT ELIGIBLE:

  • Employees of East Lake Pediatrics and their immediate family members
  • Any person under 18 years of age
  • Previous winners from the past 90 days
  1. ENTRY PERIOD
  • Monthly drawings are held
  • Entry Period: 12:01 AM on the first day of each calendar month through 11:59 PM on the last day of that month
  • Drawing Date: Within 5 business days after month end
  • Entries do not carry over to subsequent months
  1. HOW TO ENTER

Method of Entry: Complete the post-visit Patient Access & Satisfaction Survey provided via:

  • QR code at checkout
  • NFC tap at checkout
  • Text link sent to phone

Entry Limits:

  • One (1) entry per patient visit
  • Multiple visits in a month = multiple entries allowed
  • Incomplete surveys do not qualify for entry

Optional Activities: Leaving reviews on third-party sites (Google, Healthgrades, Yelp) is completely OPTIONAL and does not:

  • Impact your entry into the drawing
  • Increase your chances of winning
  • Disqualify you from winning
  1. PRIZE

Monthly Prize:

  • One (1) $75 Publix Super Markets gift card per calendar month
  • Total ARV (Approximate Retail Value): $75.00

Prize Conditions:

  • No cash substitution
  • No prize substitution except at Sponsor’s discretion
  • Prize is non-transferable
  • Lost or stolen gift cards will not be replaced
  • Gift card subject to Publix terms and conditions
  1. WINNER SELECTION

Random Drawing:

  • Conducted using random selection software or Random.org
  • From all eligible entries received during the Entry Period
  • Conducted by East Lake Pediatrics administrative staff

Winner Notification:

  • Winners contacted within 5 business days of drawing
  • Contact via phone number provided during visit registration
  • Three (3) attempts will be made to contact winner
  • Winner must respond within seven (7) days or alternate winner selected
  1. ODDS OF WINNING

Odds depend on the number of eligible entries received each month. Estimated 1 in 400-800 based on average monthly patient visits completing surveys.

  1. GENERAL CONDITIONS

By entering, participants agree to:

  • Be bound by these Official Rules
  • Release Sponsor from any liability related to the drawing
  • Allow use of name for winner announcement (first name only)

General Terms:

  • Sponsor reserves right to cancel/modify drawing for any reason
  • Sponsor not responsible for lost, late, or misdirected entries
  • All federal, state, and local taxes are winner’s responsibility
  • Prize value may be reported to IRS as required by law
  • Void where prohibited by law

Requests must be received within 60 days after the applicable month’s drawing.

  1. DATA PRIVACY

Survey Responses:

  • Used only for practice improvement
  • Handled in compliance with HIPAA regulations
  • Not shared with third parties
  • Not used for marketing without consent

Optional Reviews:

  • Posted on third-party platforms
  • Governed by those platforms’ privacy policies
  • Public information not protected by HIPAA
  1. DISPUTES

Governing Law:
This drawing is governed by the laws of the State of Florida.

Dispute Resolution:
Any disputes shall be resolved individually, without resort to any form of class action.

  1. REGULATORY COMPLIANCE

This drawing complies with:

  • Federal Trade Commission (FTC) guidelines
  • Anti-Kickback Statute regulations
  • HIPAA privacy requirements
  • Florida state sweepstakes laws
  1. MODIFICATIONS

Sponsor reserves the right to modify these rules. Modifications will be posted at [website/contestrules] and effective immediately upon posting.
